Reminder for your first visit:
Bring your referral form and any recent radiographs
Bring your dental insurance card
Bring your driver’s license or some other form of picture identification
ANTIBIOTIC PREMEDICATION: If your physician has told you that antibiotic premedication is necessary prior to dental treatment, please make sure that you have the prescription prior to your appointment and take as directed.
Financial:
We will be more than happy to submit insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want.
Forms of payment accepted by the office are check, cash, credit card (Visa, Mastercard, American Express, Discover), and we offer interest free payment options through Care Credit.
Cancellation:
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice. Short notice cancellations increase the cost of dental care for everyone.
